Seabeck 2021 Printable Registration Form

Please use the following printable registration form (PDF) to register for the 2021 Seabeck Haiku Getaway. Please print out a copy, fill it out, and mail it with your payment to the address indicated on the form as soon as you are able. After September 30, 2021, please also email Tanya McDonald at tanyamc1375@gmail.com to let her know your registration is coming. This year we can probably accommodate registrations as late as a few days before the retreat starts, but please be sure to let us know by email if you are coming. In addition, you will be required to email your registrar proof of full vaccination (scan or photo of your current vaccination card).
